In order to treat you, your practitioner will ask you to provide certain information. This includes the information you have provided overleaf and the information disclosed in your appointments. Your practitioner will add to this information details of the conditions for which you have consulted them and the remedies and other therapies that they have prescribed / recommended.

How do I collect this personal information?

All the information collected is obtained directly from you, usually during appointments but possibly also via any communications you may have outside of appointments. Your consent is required in order to ensure your practitioners compliance with UK GDPR (2022) (UK General Data Protection Regulation).

How do we use this personal information?

Your practitioner uses your personal information to analyse the conditions for which you have consulted them and to prescribe / recommend remedies and other therapies.

With whom do we share your personal information?

We do not share your personal information at all.

How long do we keep your personal information?

Your practitioner needs to keep your information for 7 years after your last consultation, in the case of children for 7 years after their 18th Birthday, after which your information will be securely destroyed.

How your information can be updated or corrected

You need to inform your practitioner of any changes you believe they should make to the personal information they hold.

How do we store your personal information?

Your contact details are held electronically and your patient files are paper-based and they are held securely within the clinic.
